Assist-Life Stage, Transition

Life Stage and Transitions is about helping you move through important changes in your life as smoothly as possible. These changes might be planned — like moving into your own place — or unexpected, like a change in health or support needs.

We provide guidance, planning and hands-on support so you don’t have to manage everything alone.

Ways we can support you

Every person’s journey is different, but we can support you with transitions such as:

  • Moving house or accommodation – planning the move, packing, organising utilities and settling into a new home

  • Leaving school – exploring options for training, work, day programs or further study

  • Starting work, volunteering or study – building routines, managing transport and balancing your week

  • Changes in support needs – adjusting your supports when your health, goals or living situation changes

  • Establishing new routines – setting up daily and weekly routines that work in your new situation

  • Linking with services – connecting you with community supports, therapists, employment services and other providers

Who Is This Service For?

This service is suitable for NDIS participants who are:

  • Moving out of the family home or into different accommodation

  • Leaving school and exploring what comes next

  • Starting or finishing work, volunteering or study

  • Experiencing changes in health or support needs

  • Feeling overwhelmed by paperwork, appointments or planning

  • Wanting help to set up new routines or supports that fit their current life

We’ll adapt the level of support to how much help you want with planning, decision-making and practical tasks.

How Our Life Stage & Transitions Support Works

    • We listen to your story and goals
      We start by understanding what’s changing, how you’re feeling about it and what you’d like life to look like on the other side of the transition.

    • We break things into manageable steps
      Together we turn the transition into a clear plan — with tasks, timelines and who will do what.

    • We support you to take action
      We can help with phone calls, forms, appointments, viewing housing options, meeting services and setting up new supports.

    • We help you settle into the change
      Once the change happens (like the move or new routine), we stay involved to help you adjust and solve any early challenges.

    • We review and refine
      We check in after a period of time to see how things are going and what might need to be changed or improved.
