Specialised Support Coordination
Expert coordination for participants with complex needs and situations.
What is
Specialised Support Coordination?
Specialised Support Coordination is a higher level of support coordination for people whose situation is more complex than usual.
It is generally funded for NDIS participants who:
- Have complex support needs
- Are experiencing significant risk or instability
- Need to work with multiple service systems (for example, health, housing, justice, child protection or mental health)
Your specialised support coordinator uses advanced skills to help you manage risks, keep important services engaged and build a more stable support network around you.
What our
Specialised Support Coordinators do
We work alongside you to:
Understand your NDIS plan – explain what each budget is for and how it can be used in your situation.
Link you with suitable services – connect you with providers who can meet your specific needs, including complex or specialised supports.
Coordinate across multiple systems – work with health, housing, justice, mental health, AOD services and others so they are not all operating in isolation.
Develop crisis and risk plans – identify risks early and create plans for how we will respond if things become unsafe or unstable.
Build a stable support network – help you put in place a support team that is reliable, consistent and able to work together.
Advocate for your needs – support you to have your voice heard with services and systems, within the scope of our role.
Prepare for NDIS reviews – help you gather reports, clarify your goals and provide information to the NDIS when your plan is reviewed.
Who Is This Service For?
Specialised Support Coordination is usually funded for NDIS participants who:
Have very complex support needs or circumstances
Are at high risk of breakdown in supports, housing, health or safety
Are involved with multiple systems or services (such as justice, child protection, mental health or AOD)
Need specialist skills to coordinate their supports safely and effectively
Have “Specialised Support Coordination” specifically included in their NDIS plan
If you’re unsure whether you have this funding in your plan, we can help you check and explain what is available.
How Our Support Works
We get to know your situation
We start by listening to your story, understanding what is happening now and what you would like to be different. We map out your supports and systems
We look at which services and agencies are already involved, where the gaps are and where the biggest risks sit.We develop a coordination plan
Together, we agree on priorities and actions – who we need to contact, what needs to be put in place and in what order. We coordinate and monitor supports
We help you link with services, share information (with your consent), attend meetings and keep track of how supports are working. We review and adjust as things change
As your situation stabilises or changes, we update the plan and adjust supports so they continue to match your needs and goals.
Our Approach
Respectful and non-judgemental
We recognise that life can be complicated, and we are here to help, not to blame.
Participant-focused
Your voice, rights and preferences sit at the centre of all planning and decisions.
Trauma-informed
We work in ways that emphasise safety, choice, trust and collaboration, and we are mindful of the impact of past experiences.
Collaborative
We bring services together wherever possible, encouraging shared plans and regular communication instead of everyone working separately.
